Frequently asked questions
What should I send with a home health referral?
Send the signed order or referral, the face sheet with insurance, the H&P or discharge summary, the current medication list and the face-to-face encounter note if you have it. If something is missing, our intake team will call your office for it.
Who can sign home health orders for Medicare patients?
A physician or an allowed practitioner, such as a nurse practitioner, clinical nurse specialist or physician assistant, can certify home health and review the plan of care. The face-to-face visit must happen within 90 days before or 30 days after the start of care.
Which areas and insurance plans do you take referrals for?
We accept referrals for patients in Hillsborough, Polk and Manatee counties. We accept Traditional Medicare, plus long-term care plans and private pay, and we verify coverage before the first visit. Medicare Advantage coverage and our participation depend on your specific plan, so we verify both before care starts.
Can families request home health care themselves?
Yes. A family member can call us and we will contact the doctor for the order. Home health still needs a doctor's or allowed practitioner's order and, for Medicare, the patient must be homebound and need skilled care.
Do you accept patients who speak Spanish?
Yes. Our clinicians and office staff speak English and Spanish, so we can do teaching, consent and phone calls in the patient's preferred language.
